Important Benefits Documents

LYNX has two medical options offered through Cigna with an optional Flexible Spending Account (FSA) through Chard-Snyder. With these medical plans, preventive care is covered at 100% with no need for copay or to reach a deductible. Medications are provided with a copay and the cost will vary depending on the tier of the medication. For all non-preventive care, a copay or deductible will apply depending on the plan selected. LYNX offers an employee only base plan free to its employees. Those employees that wish to upgrade or complete LYNX’ wellness initiatives can enroll in the enhanced plan that offers more copays and a lower deductible than the base plan. Dependents can be added at an additional cost. Each plan also has an out-of-pocket maximum (safety net) which varies depending on the health plan the employee chooses. Once the out-of-pocket maximum is met, all covered medical will be paid 100% by the plan for the rest of the calendar year.

This link leads to the machine readable files that are made available in response to the federal Transparency in Coverage Rule and includes negotiated service rates and out-of-network allowed amounts between health plans and healthcare providers. The machine-readable files are formatted to allow researchers, regulators and application developers to more easily access and analyze data. 

Flexible Spending Account

This is a pre-tax account for employees to set aside a portion of their earnings to pay for qualified out-of-pocket medical expenses. To help employees pay for medical and pharmacy expenses, FSA’s can be set up by eligible employees through Chard Snyder:

  • Money deposited can be tax-free
    For new employees, there is a 60-day waiting period.

Dental Insurance

There are three options for participation:

  • DHMO Plan - This is a plan where an employee selects a dentist from a network. The employee must go to this dentist until they designate a new dentist with provider. Preventive services are covered at 100%. All other procedures are covered by specific co-pays. There is no annual maximum.
  • Low PPO Plan - This is traditional coverage with a $50 deductible. Preventive services are covered at 100%. Basic services are covered at 70%, and major services are covered at 40%. There is a $1,000 maximum reimbursement per year, and orthodontia is limited to children, with a $1,000 lifetime maximum.
  • High PPO Plan - This is traditional coverage with a $50 deductible. Preventive services are covered at 100%. Basic services are covered at 80%, and major services are covered at 50%. There is a $1500 maximum reimbursement per year, and orthodontia is limited to children, with a $1000 lifetime maximum.

For new employees, there is a 60-day waiting period.

Vision

The Vision Plan provides coverage for routine eye examinations every 12 months. It also provides for required corrective lenses every 12 months and frames every 12 months. There is a $5 co-payment for an examination plus a $15 co-payment for frames and/or lenses prescribed when listed providers are used. For new employees, there is a 60-day waiting period.

Long-Term Disability

Disability coverage is provided at 60% of the employee’s salary, up to a maximum of $10,000 a month. This coverage is provided at no cost to the employee and benefits are available after 180 days.

Employee Short-Term Disability

Employees have the option of paying for a Short Term Disability (STD) plan. As with the LTD, the STD Plan will pay employees an amount up to 60% of their pre-disability salary to a monthly maximum of $10,000 up to 6 months. LYNX has five Short-Term Disability Plan options for employees to choose from: A 120 day waiting period; a 90 day waiting period; a 60 day waiting period; a 30 day waiting period; and a 15 day waiting period. Benefits are available after the waiting period has been met and all available leave balances are exhausted.

Employee Life Insurance

Life insurance is provided at the rate of 100% of an employee's annual salary; accidental death and dismemberment coverage is two times the employee's life insurance amount. The entire premium is paid in full by LYNX. An employee may purchase additional life insurance coverage up to five times the annual salary in increments of $10,000. There is a $500,000 limit on the total value of life insurance (basic and additional life insurance combined).

Dependent Life Insurance

Optional spouse life insurance may be purchased in $10,000 increments, not to exceed 50% of employee's total life insurance. Optional child dependent life insurance coverage may be purchased in amounts of $5,000 or $10,000, not to exceed 50% of employee's life insurance.

Retirement

Administrative employees participate in a Defined-Contribution Plan called a Money Purchase Plan. Bargaining Unit employees hired after 3/1/2014 participate in a Defined-Contribution Plan called a Money Purchase Plan.

Deferred Compensation Plan

Deferred Compensation is an optional benefit provided by LYNX to assist employees with an additional retirement savings plan. This is an employee-funded retirement plan. No federal income tax is paid on the salary deferred, or any of the investment earnings until it is drawn out at retirement. There are tax implications for an early withdrawal.

Paid Holidays

Administrative employees receive eight (8) paid holidays per year. Bargaining Unit employees receive seven (7) paid holidays per year.

Personal Leave

Administrative employees receive two (2) paid personal days per year. Bargaining Unit employees receive three (3) paid personal days per year.

Vacation

LYNX provides full-time administrative employees with paid vacation based on their years of service. The accrual rate is per administrative policy. Part-time administrative employees working at least 20 hours per week (1,040 hours per year) earn vacation on a pro-rata basis.

Bargaining unit employees earn vacation as outlined in their collective bargaining agreement.

Bereavement Leave

Employees may receive up to three (3) days of leave due to the death of a covered family member.

Employee Assistance Program

LYNX provides a prepaid professional counseling service to employees to help them with personal problems that may be affecting work performance Employees and their immediate family members may use this benefit. This service helps employees and their families deal with issues such as personal/family matters, financial debt counseling, substance abuse, etc. Any assistance received is completely confidential.

Credit Union

LYNX employees are eligible to join the Fairwinds or Orlando Federal Credit Union.

Wellness Center

LYNX’ Wellness Center is located at the LYNX Operations Center and at the LYNX Central Station. It is accessible for all employees after completing the Wellness Center forms. The Wellness Center contains exercise equipment and video viewing equipment for cardio workouts.
